The Open Enrollment deadline for a 2008 Medical Reimbursement Account, Dependent Care Account, enrollment in the Conference Health Insurance Plan, changing change the type of health insurance, or adding Supplemental Life Insurance is the end of the day on November 30. 
• Open Enrollment (OE) ends at midnight on 11/30/2007.
• The OE flyer is located at http://www.nccumc.org/treasurer/insurance.htm
• 2008 Elections should be entered on-line at www.ebenefitsnow.com
• A statement of health is required if enrolling in the supplemental life in an amount equal to or greater than $100,000
• A Statement of Health form can be obtained by contacting Caroline Thornton at 800-849-4433 extension 228.
